COVID-19 Update

COVID-19 UPDATE       October 21, 2020

Hello!  Know that we are excited to serve you.  It is of utmost importance for us to do so in a safe manner.  

There are many new precautions in place. We appreciate your cooperation ensuring that all of the necessary steps are carried out.

  • All visitors entering our clinic will be screened for COVID-19. If a patient of the clinic requires a companion, only one companion is permitted, they will be screened as well.
  • Hand sanitizer is available throughout the clinic.  We ask that you use it upon entering and as needed throughout your visit.
  • Plexi glass has been installed at our front desk.
  • We ask you to use tap as a form of payment when possible.
  • Please come into the clinic at your allotted appointment time.  If you arrive early, we ask that you wait in your vehicle to avoid congestion.
  • The Middlesex London Health Unit has a Face Covering Order in effect.

We ask that you do not enter the clinic if you are experiencing COVID symptoms. Kindly contact us by phone.

Please self screen in the following manner.  You will be required to complete the same screening when you book your appointment and in a documented manner when you arrive.

1. Have you travelled out of Canada in the last 14 days?

2. Have you tested positive for COVID-19 or had close contact with a confirmed case of COVID-19 without wearing appropriate Personal Protective Equipment?

3. Are you experiencing any of the following symptoms:

  • Fever
  • New onset of cough
  • Worsening chronic cough
  • Shortness of breath
  • Difficulty Breathing
  • Sore throat
  • Difficulty Swallowing
  • Decrease or loss of sense of taste or smell
  • Chills
  • Headaches
  • Unexplained fatigue/malaise/muscle aches (myalgais)
  • Nausea/vomiting, diarrhea, abdominal pain
  • Pink Eye (conjunctivitis)
  • Runny nose/nasal congestion without other known cause

4. If you are over 70 years, have you experienced any of the following symptoms:

  • delirium
  • unexplained or increased number of falls
  • acute functional decline
  • worsening of chronic conditions
