COVID-19 UPDATE       September 17, 2021


Know that we are excited to serve you. It is of utmost importance for us to do so in a safe manner.

There are many precautions in place. We appreciate your cooperation ensuring that all the necessary steps are carried out.

  • All visitors entering our clinic will be screened for COVID-19. If a patient of the clinic requires a companion, the companion will be screened as well.
  • Hand sanitizer is available throughout the clinic. We ask that you use it upon entering and as needed throughout your visit.
  • Plexi glass has been installed at our front desk.
  • We ask you to use tap as a form of payment, when possible.
  • Please come into the clinic at your allotted appointment time. If you arrive early, we ask that you wait in your vehicle.
  • The Middlesex London Health Unit has a Face Covering Order in effect.

Appointments may be made to see all practitioners. Virtual appointments are still available with some. You are welcome to connect with them directly through the contact information provided on their individual page on our website or by contacting 519 652-5597 or

You may book many services online at: with the exception of chiropractic care and social work

We ask that you do not enter the clinic if you are experiencing COVID-19 symptoms. Kindly contact us by phone.

Please self screen in the following manner. You will be required to complete the same screening when you book your appointment and in a documented manner when you arrive.

  1. Have you received your second vaccination dose, more than 14 days ago?

    If you prefer not to disclose this information, please answer all the questions below:

    If you answered “Yes”: to Question 1, proceed to completing Questions 2 and 3.
    If you answered “No” to Question 1, proceed to completing Questions 2, 3, 4 and 5.
  2. Are you experiencing any of the following symptoms:
    • Fever and/or chills
    • New onset of cough or worsening of chronic cough
    • Shortness of breath
    • Decrease or loss of sense of taste or smell

      Adults 18 years of age and over:
    • Unexplained fatigue/lethargy/ malaise/muscle aches (myalgias)

      Children under 18 years of age:
    • Nausea/vomiting, diarrhea, abdominal pain
  3. Have you tested positive for COVID-19 in the past 10 days or have you been told that you should be isolating?
  4. Have you travelled outside of Canada in the past 14 days?
  5. Has the person had close contact with a confirmed case of COVID-19 without wearing the appropriate Personal Protective Equipment?
